All requests for an individual’s personal information (Subject Access Requests) are processed through the NHS England Customer Contact Centre.

Requests for information from currently registered GP’s / Dentist’s or Acute Hospitals will be redirected as appropriate.

If the request is for information for an individual that is not registered with a GP or is deceased, the responsibility of fulfilling this request is NHS England’s
